Community Update: 3/11/2022
We remain committed to sharing information with you regarding the COVID-19 situation and all of the precautionary measures enacted to prevent any spread of the virus at our community.
Under reporting requirements by the state health department and the Centers for Medicare and Medicaid Services (CMS), we are providing a weekly update regarding any information as it relates to the current Coronavirus situation.
It has been our commitment to share with all residents, family members and employees if there has been a positive COVID-19 case at our community. Since March of 2020 when tracking started, our cumulative totals of as of today, March 11th, are as follows: 213 residents or employees have tested positive for COVID-19. This includes 16 residential living residents, 77 in skilled nursing, 8 in assisted living memory, 12 in assisted living and 93 employees and 7 contract workers. 24 residents, within those numbers, who tested positive have passed away.
We maintain close contact with both the county and state health departments regarding the latest information and updates, along with CMS and CDC, and per these communications our updated infection control protocols include:
- Individuals presenting COVID-19-like symptoms are immediately isolated and we following CDC and local and state health department guidelines when caring for those with COVID-19.
- For any possible healthcare resident with a confirmed COVID-19 case, all efforts will be made to provide a private room with private bathroom.
- We have primary care employees dedicated to each resident. If we have multiple residents with COVID-19 or COVID-19 like symptoms, we attempt to ensure the same nurse and CNA is assigned to all COVID-19 patients.
- All employees wear masks and those caring for COVID-19 patients wear full personal protective equipment (PPE) to ensure their safety and to prevent the spread of the virus to others.
- All healthcare resident rooms are disinfected twice daily and there are enhanced cleanings in all common areas within the health center.
- All employees have been trained in proper PPE usage, as well as proper infection control measures.
- Employee and contractor screenings continue to be required with daily health assessments and temperature screening; healthcare employees are screened at the beginning of their shifts. If anyone fails the screening, they are NOT allowed to enter.

We learned that one healthcare employee, who does not provide direct resident care, has tested positive for COVID-19. This individual is at home in quarantine. In addition, we received notification that one resident in skilled nursing has tested positive.

Under reporting requirements by the state health department and the Centers for Medicare and Medicaid Services (CMS), we are providing a weekly update regarding any information as it relates to the current Coronavirus situation.
It has been our commitment to share with all residents, family members and employees if there has been a positive COVID-19 case at our community. Since March of this year when tracking started, our cumulative totals of as of today, September 18, are as follows: 54 residents or employees have tested positive for COVID-19. This includes 0 residential living residents, 27 in skilled nursing, 2 in assisted living and 25 employees. 15 residents, within those numbers, who tested positive have passed away.
CMS guidelines also state reporting requirements of when three residents and/or employees in skilled nursing exhibit respiratory symptoms within 72 hours of each other. As of today, we have 3 residents that are experiencing respiratory symptoms.
We maintain close contact with both the county and state health departments regarding the latest information and updates, along with CMS and CDC, and per these communications our updated infection control protocols include:
- Individuals presenting COVID-19-like symptoms are immediately isolated and we following CDC and local and state health department guidelines when caring for those with COVID-19.
- For any possible healthcare resident with a confirmed COVID-19 case, all efforts will be made to provide a private room with private bathroom.
- We have primary care employees dedicated to each resident. If we have multiple residents with COVID-19 or COVID-19 like symptoms, we attempt to ensure the same nurse and CNA is assigned to all COVID-19 patients.
- All employees wear masks and those caring for COVID-19 patients wear full personal protective equipment (PPE) to ensure their safety and to prevent the spread of the virus to others.
- All healthcare resident rooms are disinfected twice daily and there are enhanced cleanings in all common areas within the health center.
- All employees have been trained in proper PPE usage, as well as proper infection control measures.

Following another round of baseline testing within healthcare, we learned that one employee, who is asymptomatic, has tested positive for COVID-19. All other results came back as non-detect, or negative.

We’ve received notification that three healthcare employees have tested positive for COVID-19. These individuals are at home in quarantine. In addition, the global testing of all residents in assisted living and assisted living memory care came back with negative results.
Our cumulative totals as of today, June 13 are: 47 cases of COVID-19 that include, 25 residents within skilled nursing have tested positive, 2 residents within assisted living and 20 employees within healthcare have tested positive. The health and safety of our residents and employees is our highest priority and our commitment is to always keep them informed of what is happening at the community—especially as it relates to the current environment with Coronavirus.
Since the last update, we learned that two residents within skilled nursing have tested positive for COVID-19. As of today, May 15, we have had a cumulative total of 12 cases of COVID-19. Eight within skilled nursing, and four employees within healthcare. Four residents (one of which was an outside admission) in skilled nursing who passed away are included in this number.
We remain in close contact with the Minnesota Department of Health (MDH) and continue to monitor all residents and employees who may exhibit any symptoms consistent with COVID-19 and, if necessary, ask that they reach out to their personal physicians for medical advice.
Our infection control protocols follow guidance from MDH, along with the CMS and CDC which include:
- Individuals presenting COVID-19-like symptoms are immediately isolated and we follow CDC and local and state health department guidelines when caring for those with COVID-19.
- For residents with confirmed COVID-19 cases, all efforts will be made to provide a private room with private bathroom.
- We have primary care employees dedicated to each resident. If we have multiple residents with COVID-19 or COVID-19 like symptoms, we attempt to ensure the same nurse and CNA is assigned to all COVID-19 patients.
- All employees wear masks and those caring for COVID-19 patients wear full personal protective equipment (PPE) to ensure their safety and to prevent the spread of the virus to others.
- All resident rooms are disinfected twice daily and there are enhanced cleanings in all common areas within the health center.
- All employees have been trained in proper PPE usage, as well as proper infection control measures.
- Employee and contractor screenings continue to be required with daily health assessments and temperature screening.

From the onset, we have worked to be diligent and open in providing information whenever we learned of new cases at our community.

Covenant Living of Golden Valley continues to navigate the ever-changing dynamics of the Coronavirus. Our employees, at every level, have been working tirelessly to maintain the care and support for all of our residents. From those within health care, dining services, housekeeping, facilities to those in administration. We cannot thank them enough for their passion and commitment to help in preventing the spread of this virus.
We have learned that there have been additional reported cases of individuals at our community who have tested positive for COVID-19. Since the last update on May 11, we learned that one more employee within healthcare has tested positive for COVID-19. Additionally, we were deeply saddened to learn that one resident in skilled nursing has passed away. Our prayers and condolences are with the family of this individual.

Our ongoing commitment is to provide residents, family members and employees with the latest information regarding the Coronavirus situation at Covenant Living of Golden Valley. Since our last communication on May 4, we are saddened to have learned that one of our skilled nursing residents who had tested positive for COVID-19 and has been on hospice has passed away. Our deepest condolences and prayers are with their family and loved ones. This individual was part of the Covenant Living family and will be missed.
To date, since April 21, we have had a cumulative total of 3 cases of COVID-19. Two within skilled nursing, and one employee within healthcare. The resident who passed away is included in this number.
We continue to monitor all residents and employees who may exhibit any symptoms consistent with COVID-19 and, if necessary, ask that they reach out to their personal physicians for medical advice.
We cannot thank our employees enough for the commitment of serving and caring for our residents each and every day. And we thank our residents and family members for their patience, support and understanding of the situation we are facing, not only here, but across the nation.

Information regarding the Coronavirus continues to evolve and we are providing our residents, family members and employees with the latest of what the situation is at Covenant Living of Golden Valley. From the onset, we have worked to be diligent and open in providing information whenever we learned of new cases at our community.
Since our last communication on May 2, we learned that an employee within healthcare has tested positive for COVID-19 and is in isolation at home. This employee has not worked at the community since last Tuesday, April 28.
The new case has been reported to local and state health authorities. We continue to work closely with them regarding guidance on infection control protocols in addition to those we already have in place.
All employees coming to work each day are screened with health assessment questions and a temperature screening. If an employee, visitor or contractor fails in any area of the screening, they will NOT be allowed to enter or will be sent home.

Covenant Living of Golden Valley learned today that an outside admission patient within skilled nursing has been diagnosed positive with COVID-19. This individual is currently in isolation. At this time there are no other COVID-19 positive cases at our community.
As a matter of protocol, we have been in contact with local and state health authorities and will be working closely with them regarding additional guidance on infection control protocols, in addition to the stringent measures we already have in place. We also have notified any employees and residents who may have been in close, prolonged contact with this individual and will be monitoring them for any indication of symptoms as per CDC guidelines.
In following our current guidelines and protocols, all employees coming to work each day are screened with health assessment questions and a temperature screening. If an employee, visitor or contractor fails in any area of the screening, they will NOT be allowed to enter or will be asked to go home.
We continue to monitor all residents and employees who may exhibit any symptoms consistent with COVID-19 and, if necessary, ask that they reach out to their personal physicians for medical advice.

Covenant Living Communities and Services continues to stay on top of the latest regarding the situation with the coronavirus (COVID-19) across the nation and in our local communities. To date, none of our communities have been directly impacted.
However, due to concerns regarding the spread of coronavirus and in order to maintain a healthy community, we respectfully discourage all non-essential visits to our community. This request is for all individuals—including family members—as a preventive measure and precautionary step to protect our residents and staff until further notice.
Covenant Living is already following strict protocols and guidelines established by the Centers for Disease Control and local and state health departments. Our emergency response plans include everything from infectious control protocols, ensuring our health care employees providing care have adequate supplies to contingencies regarding dining services and activities, if necessary. In addition, we are reinforcing regular and thorough hand washing as recommended by the Centers for Disease Control and telling any employee who is sick to stay home.
It is important we all remain vigilant and pay close attention to our own health and places we visit, in particular if any resident, employee or visitor has recently returned from travel overseas or has any signs of illness.
